Medical Records

The Health Information Management Department at Scott & White Hospital - Brenham is dedicated to safely maintaining your medical records and keeping your health information private.

Copies of your own medical records, those of a minor child, or the medical records of someone for whom you are the legal guardian or hold a Durable Power of Attorney for Healthcare may be requested. Proof of legal guardianship or a Durable Power of Attorney for Healthcare is required for access of the records.

To obtain copies of your medical record, contact our Health Information Management Department (Medical Records Department) at 979-830-7430 during regular business hours, Monday - Friday 7:00 am to 5:00 p.m. To obtain a copy of your medical records, please print out a copy of our Authorization for Release of Medical Information form. Please complete the form and deliver it to Scott & White Hospital - Brenham Health Information Management Department, fax to 979-830-7429 or mail to the following address:
