Quality & Safety ScoresScott & White Hospital, Brenham (SWB) is committed to providing safe quality care in the most efficient and effective way. Our physicians, administration and healthcare team work together to always provide you and/or your loved one excellent care. Our goal is to assure that those who entrust their care to us are confident that our focus is on what is best for our patient and their family. We constantly seek ways to improve the processes within our hospital. Our aim is for our patients and their families to be satisfied with the care received at SWB. Our hospital is accredited by The Joint Commission (TJC) with a deemed status by the Centers for Medicare & Medicaid Services (CMS). The expectations of TJC and CMS provide a framework establishing standards through which our policies and procedures are developed. This guides our practice and enables us to measure our processes in the delivery of safe patient care. Performance Improvement is a collaborative effort between our physicians, administration, and the healthcare team as we strive to improve our processes and provide quality care. Each of us is held accountable for our practice and patient safety. We know that people do not choose to get sick and would rather not be in a hospital. Yet, at one time or another it happens and we do need to be admitted. It is important to us that you are as comfortable as possible in our setting. Our healthcare team works together in hopes of making you feel safe while you are here. One way of helping you feel comfortable and safe is for each of us to acknowledge you in a caring, compassionate way; introduce ourselves and let you know a little bit about our ability to do our jobs; the duration and explanation of tests and procedures that have been ordered, and keep you updated if timeframes change; listen to you and answer your questions; let you know who your caregivers will be when our shift is over and thank you for sharing your thoughts, concerns and for trusting us to provide your care. We encourage our patients to "SPEAK UP". Become actively involved in your healthcare. We strive to have processes in place to prevent mistakes from occurring, however we are human and sometimes mistakes happen. YOU can help us prevent this! Ask questions of anyone - ask about your medications, treatments, and procedures. Your nurse, respiratory therapist, lab technician, and social worker should be asking you specific identification information every time a medication, treatment, or lab work is done. You may get tired of being asked your name and date of birth; yet, remember, YOU are special and we want to make sure that the care being provided is for the right person at the right time! Again, our AIM is to provide you with safe care every time. YOU and your FAMILY are vital to this process. Feedback is an important measurement of evaluating how we do in "always" providing safe, quality care. Randomly, patients are selected to provide feedback via telephone on "how we are doing." This information is summarized by an external vendor. Your opinions are taken seriously, are reviewed and help us identify opportunities that may need to be revised or changed to improve our practice for the better.
